Company name The name you go by. This is what we will call your account.

Legal entity name (optional) Only if the registered name that should appear on invoices differs from the company name above.

License number(s) All licenses that require testing. Separate them with commas.

Primary facility address This is where we will collect samples.

Expected samples per month A rough figure is fine. It helps us plan capacity, and nothing is held to it.
